If you're aged 12 or under:Your parent/carer must complete this form using their own details. We are unable to collect personal details from children under 13. If you are attending an iDiscover activity, your parent/carer can provide your details when booking onto a course
If you're aged 13 to 15: You can complete this form yourself, but you must confirm that you have consent from your parent/carer. You will also need to provide their contact details.
If you're 16 or older (including parents/carers/professionals): You can complete this form for yourself and give your consent to signing up

Important: If you're a parent/carer and you wish to bring a child aged 12 or under to an iDiscover activity, you will be asked to provide their details when booking a session.
